Outbound and inbound change sets are the same metadata package seen from opposite orgs: outbound is what you build and send from the org you are logged into, and inbound is what the destination org receives. Upload only delivers the package; the destination must still deploy it before any customization takes effect. Administrators usually send tested sandbox work outbound to production, then deploy it inbound there.
