Milestone actions are time-dependent workflow actions that run on each milestone in an entitlement process so the org can notify people, update the support record, start a flow, or send data outside Salesforce when a service timeline is near, missed, or completed. Warning and violation actions require their own time trigger before you can attach a workflow action; success actions use the milestone's time trigger instead. You add them on the milestone after that milestone is already on the entitlement process.
