Each contact still has one parent company in the Account Name field: that parent is the primary account, and the relationship is direct. Contacts to Multiple Accounts exists so the same person can also be tied to other companies as indirect relationships, without duplicating the contact. Extra ties are stored as Account Contact Relationship records, while most Salesforce features still treat the Account Name parent as the account that matters.
