When you install a package, you choose which users can access its functionality, and Salesforce writes matching access onto profiles. In Enterprise, Unlimited, Performance, and Developer Editions the prompt offers Admins Only, All Users, or Specific Profiles. The choice lets you roll a package out narrowly or broadly at install time instead of giving every user the same access.
