Change sets move Setup customizations from one Salesforce org to another as metadata, not records. From the org you are logged into, an outbound change set is the package you assemble and upload; an inbound change set is that same package after another org has sent it to you. Upload alone does not change the destination, the receiving org must still deploy the inbound set before any customization takes effect.
