An AppExchange package is a container of apps, tabs, and objects that you add to a Salesforce org. You choose the target environment, production or sandbox, when you install, and experienced admins test in a sandbox or Developer Edition first so an unexpected install does not affect live users. The provider ships either a managed or unmanaged package, and that choice controls whether you can change the solution, how upgrades arrive, and whether the contents consume your org's app, tab, and object limits.
